Nu utilizați allopurinolul dacă sunteți alergic la allopurinol sau la orice alt ingredient din acest medicament.

Indicații

Allopurinolul este folosit pentru a trata podagra la adulți prin gestionarea atacurilor acute, deteriorării articulare și problemelor de sănătate cauzate de acumularea acidului uric. Este folosit și la pacienții cu cancer care primesc chimioterapie pentru a preveni creșteri periculoase ale nivelului acidului uric. De asemenea, ajută la prevenirea calculilor renali recurenți la pacienții cu excreție foarte ridicată de acid uric.

Contraindicații

Nu utilizați allopurinolul dacă sunteți alergic la allopurinol sau la orice alt ingredient din acest medicament.

Efecte secundare

Cel mai frecvent efect secundar la începerea tratamentului cu allopurinol este creșterea atacurilor de podagră, deși aceasta a devenit mai rară cu abordările terapeutice contemporane. Majoritatea pacienților tolerează bine medicamentul, iar efectele secundare grave sunt rare.

Dozare

Pentru tratamentul podagrei, doza inițială este de obicei 100 mg zilnic (50 mg dacă aveți probleme renale), crescând cu 100 mg săptămânal până când nivelurile acidului uric scad la 6 mg/dL sau mai puțin, cu o doză zilnică maximă de 800 mg. Înainte de a începe tratamentul, medicul dvs. trebuie să verifice funcția renală și hepatică și numărarea sângelui.

Mecanism de acțiune

Allopurinolul funcționează prin blocarea xantinului oxidaz, o enzimă care transformă purinele în acid uric. Prin încetinirea acestei conversii, allopurinolul reduce nivelurile acidului uric în sânge și urină, ajutând la prevenirea atacurilor de podagră și problemele renale fără a bloca producția de purine în sine.

Sarcină și alăptare

Sunt disponibile informații limitate despre utilizarea allopurinolului în timpul sarcinii. Studiile experimentale pe animale sugerează riscuri potențiale pentru fatul în dezvoltare, iar medicamentul poate traversa bariera placentară. Discutați cu medicul dumneavoastră dacă beneficiile luării allopurinolului depășesc riscurile potențiale în timpul sarcinii.

Farmacocinetica

Când se ia pe cale orală, allopurinolul este absorbit și convertit în oxipurinol, forma sa activă. Corpul distribuie atât medicamentul original, cât și metabolitul său activ pe întreaga țesut. Oxipurinolul are o durată mai lungă de acțiune și este eliminat în principal prin rinichi.
